Text Box: Olympic Labradoodles Health Guarantee and Sales Contract.  Olympic Labradoodles guarantees that at the time of puppy pick-up/arrival that the puppy is in good health.  We offer a 5 day general health guarantee and ask that the puppy buyer take the puppy to their vet (buyer’s expense) within 3 days of receipt of the puppy, for a puppy well check.   Prior to delivery we will have all puppies checked at our vet, including shots and de-worming.  We stand behind the health of our puppies.  During the buyer’s puppy well-check should it be determined that the puppy has a serious or chronic health condition the breeder shall be notified immediately, and arrangements will be made to promptly return the puppy. Once the puppy has been returned, the buyer will be provided with either a refund of the purchase price or a replacement puppy, at the breeder’s discretion.  Alternatively, the buyer may choose to keep the original puppy and Olympic Labradoodles will reimburse veterinary costs incurred in treating any pre-existing conditions, up to the puppy purchase price.  Reimbursement will require that the buyer provides the breeder with veterinary records and receipts related to the treatment. Breeder reserves the right to request a second opinion from an accredited veterinarian of our choosing at our expense.  Failure to take your puppy to an accredited vet within this 3 day time period voids this part of the health guarantee.  Olympic Labradoodles guarantees their dogs for 3 years against all life-threatening/altering genetic diseases. If the dog is determined by at least 2 veterinarians to have an inherited illness that shortens or greatly alters the quality of the life of the dog then Olympic Labradoodle will offer to reimburse the purchase price of the dog or reimburse the buyer for vet costs up the original purchase price or offer a replacement puppy.  Breeder reserves the right of choice.   There is no need to have the dog returned to Olympic Labradoodles unless the buyer feels they can no longer keep or care for the dog and wish to have the dog returned.   Olympic Labradoodles will pay for shipping costs to have the dog returned to them or assist in the negations of finding an acceptable new family.  Olympic Labradoodles guarantees for up to 3 years against HD.  Should it be determined that the dog has HD we require proof in the form of documentation from Penn hip, OFA, or BVA.    Breeder reserves the right to obtain a second opinion at their expense before any reimbursements are made or replacement puppies given.  This warranty becomes void should a dog be allowed to become overweight or the dog engaged in activities, prior to one year of age, that over stress developing joints and bones, such as jogging, excessive walking on concrete, excessive stair climbing, or other dog activities such as agility, fly ball, or Frisbee catching.  Olympic Labradoodles recommend for the pups first year that you do not go jogging or running with your pup, limit walks to a short duration and allow the pup to stop and go at will.  Limit stairs especially during the first 6 months as this can stress the developing joints.  Show care in play activities such as ball throwing, tug of war, and Frisbee catching.   This guarantee does not cover health problems attributed to abuse, trauma, or failure to care for the dog in a humane way.  All guarantees are voided should it be discovered and proven the dog is being abused.  The Breeder reserves the right to pursue actions to have the dog returned to Olympic Labradoodles in this case.  Under these conditions the buyer agrees to relinquish the dog.    Buyer agrees to return the dog to Olympic Labradoodles should it be determined that the buyer can no longer keep and care for the dog.  Buyer agrees to contact Olympic Labradoodles should they find a suitable home or if they need assistance in negotiating a new family for their dog.  Under no conditions should the dog ever be turned over to a Humane Society or Dog Shelter.   Olympic Labradoodles will reimburse the buyer 1/2 of the purchase price if returned the first year and ½ the purchase price up to 3 years if Olympic Labradoodles deems their reasons for not keeping the dog are valid.  Further negations of refunds are at the discretion of Olympic Labradoodles.  Veterinary Records must be provided if requesting a refund.  Buyer agrees to provide the dog with a loving home and to keep the dog in good health.  This includes regular veterinary care, regular vaccinations as recommended by the buyer’s vet, control of internal and external parasites, and other preventive care.   If such care is not provided the health warranty is void.  Olympic Labradoodles encourages that buyers take their puppies to training classes.  Olympic Labradoodles and Buyer agree that this agreement is between them and no other representatives and by signing agree to all the conditions set forth in this agreement.  This health guarantee and sales contract is for: Puppy whelped from (Dam/Sire):  __________________________________________________________ Whelped on:   ______________________   Sex:  _____________  Color: __________________________  Microchip:  ____________________________  Olympic Labradoodles signature _______________________________  Date _______________________  Buyer agrees to sign and return this Health guarantee (we will provide a second copy for your records) within 14 days of the puppy’s pick-up/arrival date.   Failure to do so will void this guarantee.  Buyer’s Signature _____________________________________________  Date _____________________
